We have provided 2 ways to register with the American Society of Lap Dogs™.

Click the link below to open the Membership Application in Adobe Reader.  The form may be filled out online, and then printed.  The completed form should be signed and mailed, along with a recent photo of the applicant, and your check or money order, in the amount of $9.95 to:

If your Application for Membership is not approved for admission into the Society your check will be returned un-cashed.
